DESCRIPTION:Messaging around Survey:  \nAs a provider that serves individuals 55 or over in the Mat-Su\, you are being invited to participate in a survey conducted as part of the Mat-Su Health Foundation’s Healthy Aging Initiative.  This survey is designed to better inform the community where there are opportunities to increase awareness of existing resources\, identify information and referral opportunities\, strengthen collaboration\, and identify short- to long-term goals for improving the holistic care infrastructure for adults 55 and over aging in place in the Mat-Su.  This survey is expected to take 8-10 minutes to complete depending on how readily accessible the information is to you. We have included a PDF with all the questions to help you pull all the information ahead of time. \nWe appreciate your participation and will share the results of this survey when they are available.
